Read instructions completely before attempting to collect your sample.
Step 1
- Complete the Lab Identification form
- Return White copy with blood sample
- Keep Yellow copy
- Wash and Dry Your Hands
Step 2
Open Reagent Vial and place in the Vial Holder located on the Workstation
Step 3 (top image)
- Locate the Orange Single-Use Lance Device
- Remove the clear protective cap
- Place the RED portion of the lancet on the desired puncture site
- Press device firmly against finger to activate
- Squeeze finger to Obtain a Moderate size drop of blood
Step 4 (middle image)
- Hold the capillary tube device horizontal to the finger
- Place the tip of the Tube Device into the blood sample
- DO NOT SQUEEZE THE DISPENSING BULB DURING THIS STEP
- CAPILLARY ACTION will automatically draw the BLOOD sample to the Fill Lines and stop
- If the device does not fill to FILL lines, acquire additional blood and repeat step
- The test does not require more than 1 small drop of blood
- DO NOT DROP BLOOD DIRECTLY INTO VIAL unless instructed by technical support
Step 5 (bottom image)
- Position the Capillary Tube Tip in VIAL
- SQUEEZE AND HOLD PRESSURE ON DISPENSING BULB TO EXPEL BLOOD WHILE REMOVING TUBE
- Otherwise, blood and solution will be drawn into the dispensing bulb
- If this happens, squeeze the bulb again while maintaining pressure during removal
- CLOSE SAMPLE VIAL TIGHTLY
Step 6
Place SAMPLE VIAL in vinyl bag and seal
Step 7
Place completed Lab ID Form and vinyl bag containing Sample Vial in Workstation
Step 8
Place Workstation (Arrow First) into the self-addressed/postage paid MAILER
Step 9
Peel self-adhesive tape from MAILER FLAP, tuck and close flap
