Loss of Sensation Test for the Foot

• To be used in conjunction with ongoing care by a physician
• An important test to track the progression of loss of sensation in the foot
• Meets HEDIS Outcomes Screening/Testing Criteria

Instructions for use

• Have someone else perform the test
• Hold the filament by the paper device
• Touch tip of the filament to the 12 numbered gray sites on each foot
• Hold filament to spot for 1-2 seconds, pushing gently making the filament bend
• In each numbered cirlce place a (+) if you feel the filament or place a (-) if you can not feel it
• After completing the test, place card in the envelope are return then mail card.
• Record the date the test was completed along with the ID number
